Goblin Camp: In the Shattered Sanctum of Goblin Camp, there is an NPC named Dror Ragzlin. After defeating him you can get the key from his corpse. This key gives you access to the treasure hoard room. The room is actually located right behind the throne of Dror Ragzlin.
Romancing with Karlach and First Kiss (Act 1)
Once you have got the Infernal Iron, return back to the Dammon. Handover him the Infernal Alloy. Now, Karlach can get close to anyone but this only happens for a short time. After getting the Infernal Iron, it’s time to meet with Dammon again in the Last Light Inn. After that try kissing Karlach to check that her Infernal Engine is working fine or it still has some issues.
Now, return back to your camp to speak with Karlach during the nighttime. Choose the right dialogues to start romancing with her but as of a recent update, some bugs cause issues at this point.
If you get any issues at this point, you can simply head to the Last Light Inn with Karlach recruited in your active party. Upon reaching the Last Light Inn, Karlach will mention Jaheria in her conversation. Simply just talk about Jaheria without speaking to any other NPC.
Also, look for Dammon in the stables while you are in the Last Light Inn but don’t talk to him. Next, head back to your camp and a scene between Wyll and Mizora will trigger. Once the scene is over, talk to Karlach about this. After that, get to the Dammon and him over the Infernal Iron that’s going to fix Karlach’s heart.

Don’t Let Dammon Die: you have to keep the Dammon alive no matter what. Otherwise, you won’t be able to do romance with Karlach. There will be a few discussions where your decisions can lead to Dammon’s death. Such as when you interact with Nightsong.
Moreover, try your best to save Strange Oxen in Druid Grove as well. The death of Strange Ox might cause troubles in romancing with Karlach.
As far as romance is concerned, you can also try romancing with Shadowheart. But the consequences are obviously different and the party members will also react differently to the entire scenario.
