It is located in the southern corner of the open world map, and you can get there either from Dry Steppes or by crossing the Hawezar region. Alcarnus Stronghold:
Recommended Level: 40+
Location: East of the Imperial Library, the eastern portion of the Kehjistan region.
Boss: Rashta Reborn

Alcarnus sounds like an ancient city of the Arabian Peninsula, and it lives up to its name. This ancient city has been long abandoned, and its residents have migrated to other regions. However, the empty streets of Alcarnus have been taken over by a mad Necromancer who, in search of immortality, has created countless clones. So, your objective is to investigate the source of corruption and stop the mad scientist Rashta before things get further out of hand. You can easily reach the city by going east from the Imperial Library Waypoint, as shown in the picture:
- As soon as you enter the city, you will get attacked by a few foes guarding the Necrotic Masses.
- You can defeat the attackers, but the Necrotic Masses will have a greyed-out HP bar. So, leave them for now and further investigate the site.
- Head to the northern corner of the city, where you will find the Lair of the Witch (check your map).
- Go inside the Lair and speak with Rashta, who completes a spell and attacks you immediately.
- However, the fight will be over quickly as the witch will conjure a shield and disappear when her HP reaches 10%.
- Next, interact with the medical notes and read the Research Notes (check your map) in the Lair, and return to the place where you left the Necrotic Masses.
- Now you are free to attack and destroy all 3 Necrotic Masses; beware of Rashta’s Clones and the hordes of enemies that come to protect them.
- After destroying all Necrotic Masses, the progress bar will fill up, and you will be ready to face Rashta one final time.
- When you approach the Necrotic Mass inside the Lair of the Witch, Rashta will burst out from it as Rashta Reborn.
- Finally, after defeating Rashta Reborn, go outside the Lair of the Witch and light up the Wanderer’s Shrine to liberate the Alcarnus Stronghold.
Omath’s Redoubt Stronghold:
Recommended Level: 40+
Location: East of the Gea Kul Waypoint, Southern portion of the Kehjistan region.
Boss: High Priestess Hadar

Unlike most strongholds in Diablo 4, the Omath’s Redoubt Stronghold has a rather tragic and heroic past. Instead of being taken over by evil forces, it is based on a backstory of a great sacrifice. You will eventually learn everything about this mysterious place when you solve its puzzle.

With no other option left, the Head Priestess, Hadar, sacrificed herself and all the city’s inhabitants to perform powerful magic and destroy the attackers. - The stronghold’s main entrance is blocked by a fallen cliff and debris. So, you must climb the cliff in the northwest and then jump into the stronghold.
- The game doesn’t tell you exactly what happened here and how to solve the mystery.
- Instead, you will get 4 circular markers on the map, indicating that you are seeking 4 different objects. Furthermore, you will face hordes of enemies and elites in these locations. Here’s a list of everything that you must interact with:
- First, you must find a Kehjistani’s corpse and interact with it to get the Leaflet. (North)
- Secondly, you must find and collect a Rusted Blade filled with dust. (West)
- Thirdly, you must interact with the Zakarumite’s corpse. (East)
- Lastly, you must find and interact with another Kehjistani Corpse. (South)
- After collecting all 4 clues, you will find a new marker in the center of the stronghold.
- Head to the new location and inspect the Ceremonial Staff of the petrified priestess.
- Doing so will show you a memory of how the city was saved, and the battle with the Head Priestess Hadar will begin shortly afterward.
- Finally, after defeating Hadar, head to the western section of the location to light up the Wanderer’s Shrine and liberate Omath’s Redoubt Stronghold.
Altar of Ruin Stronghold:
Recommended Level: 45+
Location: East of the Amber Sands, Northwestern portion of the Kehjistan region.
Boss: Dark Cardinal Maldul

- Unlocks the Sepulcher of the Forsworn dungeon
- +20 Kehjistan Renown (Waypoint Unlocked)

The Altar of Ruin Stronghold used to be a famous cavern where travelers stopped to rest from time to time. However, it was taken over by Khazra and the worshippers of demons. From that point on, it became a stronghold of evil forces, and anyone who came by was cursed to join their groups. So, your objective is to investigate this camp, figure out the source of corruption, and eradicate all traces of demonic worshipers. You can find this cavern by going east of the central part of the Amber Sands, as shown in the picture:
- As you approach the cavern’s main entrance, it will be blocked off by debris. So, you must find another way to enter the stronghold properly.
- You will find a ledge on the top left side of the main gate, climb the ledge follow the path, destroy the pile of rubbles, and jump to the other side to enter the stronghold.
- Next, kill the horde of cultists along with Thundering Moon Clan Mauler, a Khazra Elite, to make your way forward.
- At this point, you will learn about 3 Elite Cultists who hold the key to the Ritual Chamber. So, you must slay them all and collect their Ritual Chamber Keystones. Here’s how to find them all:
- After clearing the first area, go to the southeastern section of the cavern to face Heinrik Frostburn. However, you can’t attack him until you slay all Triune Worshippers nearby. Defeating Heinrik will drop the first Ritual Chamber Keystone.
- Next, go northeast from the previous location until you are ambushed by a large group of cultists. After dealing with them, take the northwestern path to face Orfeo Doomtrap; killing him will drop the second Keystone.
- From here, take the western path to face Sisiarh Doomblood, surrounded by a huge horde of cultists. Killing this Elite will drop the third Ritual Chamber Keystone.
- Collecting all 3 Ritual Chamber Keystones will fill up the progress bar, and now you are ready to face the final boss of the stronghold.
- So, return to the place where you killed Orfeo Doomtrap and enter the large ornate door behind him; clear all the enemies that ambush you from the ceiling.
- Next, use the Ritual Chamber Keystones to enter the arena and face Dark Cardinal Maldul.
- Finally, after defeating Dark Cardinal Maldul, head back to the location where you killed Thundering Moon Clan Mauler to light up the Wanderer’s Shrine and liberate Altar of Ruin Stronghold.
