Shadowheart has her parents in the House of the Grief. But, they are locked up deep down in a dungeon. You have to find them and make the choice of either killing them or saving them. Her parents are actually imprisoned by the Goddess Shar.
Saving Shadowheart’s Parents
This decision is best if Shadowheart is not a follower of Selune. That’s because if Shadowheart doesn’t kill her parents and chooses to spare their life then the Shar’s Curse won’t lift up and she will have to live with it.
However, her parents will always try to comfort her. This will bring a bit of change in Shadowheart’s character but the Shar’s Curse will always be there. Even if you ask her parents after they are rescued, they will mention that they prefer to be dead. And her father wants her to be freed from Shar’s Curse.

This choice in my opinion is way better than letting her parents live. As we know Shadowheart is trapped by the Shar’s Curse and this creates a lot of hurdles for her, by letting Shadowheart kill her parents, she will become free from the curse forever.
Even Shadowheart’s parents want to be killed. I know this sounds weird but, they know that this is the only way of freeing their daughter from that devilish curse.
Moreover, once her parents are dead, they become Moonmotes. They will then forever be with Shadowheart in the same form and keep supporting her spiritually.
Now, at the end, if you try to romance with Shadowheart, she will mention the guilt she has for killing her parents. She will also mention that she would have given anything just to speak with them again.
Is There Any Way to Save Shadowheart’s Parents?
Yes, there is an option that can save Shadowheart’s parents. During the dialogue scene, you will get a choice “Do not lose your parents, not again.”, with this option Shadowheart won’t kill her parents.
Both Shadowheart and her parents will stay alive. But, you have to pass a DC20 or Persuasion check. Once you successfully pass it, her parents will then head to your camp where they stay forever.
How Do You Find Shadowheart’s Parents?
Shadowheart’s parents are imprisoned in the lowest dungeon of House of the Grief. Once you are in the House of the Grief, look for the Black Diamond which is just in front of the altar. Pick up that Black Diamond and a plate will be raised.
Now, you have to interact with it and use a Silver Key. This unlocks a door to a sanctum where you will see Shadowheart’s parents chained up in the air.
