The Defiled Temple is one of the entrances to the Underdark. This temple is located on the West Side of Shattered Sanctum which is in the Goblin Camp. To make your way to the Underdark, you will need to face off with the group of goblins and also solve a puzzle.

You can either choose to convince the goblin’s group that you are not their enemy or you can kill all of them. Once you are free from them, unlock the door with a lockpick to enter the Priestess Gut’s room.

There will be her guard in that room. You have to sneak behind him and look for the Defiled Temple puzzle. Upon successfully clearing the puzzle, a doorway will be opened at the end of the temple. Entering the doorway will take you to the ladder that leads down to the Underdark.

Riverside Teahouse

This entrance is actually based on one of the quests of Baldur’s Gate 3 which is Save Mayrina. You have to complete that quest and then make your way into the Riverside Teahouse which leads you to the cave and then finally to the Underdark.

Once you have completed the quest, look for the fireplace in the Riverside Teahouse, there will be an illusory wall behind it. It will take you to the other side. Here, look for the Whispering Mask on the table.

Using this mask will result in possession. So, cast the spell Protection from Evil and Good before equipping the mask. Now, get through the wooden illusory wall that has the face on it.

In the next room, you will see Hag’s thralls. You can either fight them or sneak behind them to get to the next room. Get across the waterfall and use the ladder to climb down to the north side while looking for another wooden wall.

Here, you will again need to use the mask to get through the illusory wall to head to the secret room. This room will have a circle of mushrooms. By activating the circle you will enable the teleport that will take you to the Underdark.

The Whispering Depths

This well takes you to the pit which is the entrance to the Underdark. But, there are lots of spiders protecting the area and also the Phase Spider Matriarch which is the final boss down there. Clear out the place from the boss and all the other spiders to get access to the giant pit and finally to the Underdark.

Zhentarim Hideout

This is the last entrance to the Underdark. However, it is kind of the easiest one as you don’t have to be involved in any sort of combat. But still, you need to make your way through different rooms to get to the hideout.

This hideout is located in Waukeen’s Rest which is on the west side of Risen Road Waypoint. In the Waukeen’s Rest, look for the burning inn. Go past it and on the left side you will see a covered bridge that will lead you to a road that has a dead Ox on it.

Head to the north side from here and look for a shed that has its doors hidden behind the crates. Put away the crates and enter the hideout. Keep going down the hideout until you see a wardrobe. Unlock the wardrobe with a lockpick and keep moving.

While moving downwards, you will get a part perception check near the rock wall. On successfully passing the check, an illusory door will appear. Enter through the door and use the lockpick on the elevator. This will take you to the Underdark.

These are all the entrances to the Underdark. Once you head to the Underdark, you will get to see new NPCs and other creatures. Also, the Underdark has its own questline that you can start.
